SWEETGRASS provides integrative and compassionate support for individuals and families navigating complex mental health, addiction, and co-occurring challenges. We recognize that many people seeking help have often moved through multiple systems, diagnoses, treatments, crises, and difficult life experiences while still feeling misunderstood, overwhelmed, disconnected, or stuck. Our approach is grounded in the belief that complexity requires thoughtful, relational, and coordinated care rather than fragmented or one-dimensional solutions. We aim to create spaces where people feel supported, respected, empowered, and capable of moving toward greater stability, connection, and hope.
Clinical Case Management
Clinical case management provides coordinated and individualized support for people navigating complex mental health or addiction. Many individuals and families experience significant stress trying to manage multiple systems, providers, crises, and responsibilities at once. Our role is to help bring greater clarity, structure, advocacy, and continuity to the process. This work may involve assessment, care coordination, treatment planning, collaboration with healthcare providers and community resources, system navigation, family support, referrals, crisis planning, and ongoing therapeutic guidance. We work collaboratively with individuals and families to develop practical and realistic pathways that support stabilization, recovery, and long-term well-being.
Compassionate Intervention
Compassionate intervention services support individuals and families facing high-risk situations related to addiction, mental health, crisis, or severe life disruption. Unlike confrontational or shame-based models, our approach emphasizes dignity, relationship, understanding, safety, and collaborative engagement. Interventions are approached thoughtfully and strategically, recognizing the emotional complexity that families and loved ones often carry when trying to help someone in distress. This process may include family consultations, structured intervention planning, emotional support, treatment coordination, recovery planning, and guidance through difficult decisions and transitions. Our goal is not simply to push someone into treatment, but to create opportunities for reconnection, stabilization, and meaningful movement toward recovery.
Addictions Recovery Support
We support individuals experiencing challenges related to substance use, behavioural addictions, relapse cycles, emotional regulation, trauma, co-occurring mental health concerns, and recovery transitions. Our approach integrates therapeutic support, relapse prevention, emotional processing, practical life strategies, systems awareness, and resilience-building within a compassionate and non-judgmental environment. We work collaboratively with individuals to help them strengthen insight, develop healthier coping strategies, rebuild connection, and create sustainable pathways forward. We offer individual recovery support and outpatient recovery support groups.
